We want to hear from you. To help resolve your concerns, complaints or questions regarding university policies, or actions, please complete the following form. If you have IT questions or concerns, please contact the USD service desk using Coyote One Stop.

We begin the process of complaint resolution within 48 hours of receipt of this form. We strive to resolve complaints within 30 days of submission, though some policies and procedures may require an extension of that time frame.
